- Provides psychosocial assessments, diagnosis and treatment for patients in their place of residence as prescribed by a physician, to assist the patient/family member(s)/caregiver(s) in developing effective coping skills in response the patient's diagnosis, treatment and/or life situation.
- Performs accurate and comprehensive assessments assessing psychosocial status
- Ensures that changes in patient's clinical status are communicated to other healthcare team members and to the agency office staff verbally and writing in a timely manner.
- Coordinates care planning and delivery with the physician, patient and family/caregivers, other healthcare team members and Agency staff to facilitate optimal patient outcomes
- Educates patients, their families/caregivers and other staff as appropriate, in medical social work modalities and interventions
- Provides medical social work modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
